Health Contests 2009
This is information on the "Win With Good Health Program" of "I Can Take Charge of My Health" for 2009, from Deaf Access.

You can win prizes while participating in programs to improve your health:
1. Just fill out an entry form with your contact information, return it to Dr. Ebert by June 30, 2009, and you will be entered in a drawing for a $25 prize. Entry forms are with the Spring, 2009, Deaf Access Health Newsletter.
2. If you come to 3 or more of the "I Can Take Charge of My Life" classes in 2009, you will receive a free folder with a personalized plan for your health, or a gift of similar value if you've won a folder before.
3. There will be a drawing for one $50 gift certificate selected from the people who attend the most classes in 2009.
4. Read each newsletter, and take the quiz at the end of the year. Those with the highest test scores will be entered in a drawing for $40 in prizes.
5. New ways to win! Participate in the "Exercise" and "Eat Your Vegetables!" programs. Fill out the forms for these contests. When you have entered each line, submit each form, to be entered in one of the drawings. No limit on the number of complete, separate entries per person.
6. More prizes for more people may be added to any of these contests, based on availability, so enter now!
Rules for "Win With Good Health", "I Can Take Charge of My Life" Programs:
1. There will be only one winner of the grand prize for attending the most classes. There will be only one winner for the Deaf Access Newsletter Health Quiz grand prize. There will be only one winner for the grand prize drawing from trhe returned entry forms. There is no limit on the number of winners for attending at least 3 classes in the year, but only one folder or prize per person. Additional prizes which may be offered are at the discretion of the Deaf Access medical director and based on availability.
2. Only one test and one entry form per person.
3. Completed entry forms must be received by June 30, 2009. Drawing will be no later than August 31, 2009 for the returned entry forms.
4. Employees of Access Community Health Network and Sinai Health System are not eligible.
5. Deaf Access will publicize the names of winners.
6. Winners need not be present at drawings, but must provide information so they can be contacted.
7. The drawing for those attending the most classes in 2009 of the "I Can Take Charge of My Life" series will take place no later than the first class in 2010, or no later than May 31, 2010. At least 2 names will be in the drawing.
8. The newsletter tests must be received no later than the return date on the test, and the drawing for the highest test scores will take place no later than the first class for 2010, or no later than May 31, 2010. At least 2 names will be in the drawing.
9. Deaf Access is not responsible for any lost or delayed submissions or entries.
10. Prizes of same value may be substituted at the discretion of Deaf Access.
11. Anyone is eligible to enter the programs and contests.
12. Additional rules for the 2009 "Exercise" and "Eat Your Vegetables!" programs are in the spring 2009 newsletter and the forms for the 2 programs. You can enter these contests more than one time.
13. Rules for other contests for 2009 may be announced later.
14. In case of any disagreements, the office of the Medical Director of Deaf Access will have final determination.
