Note:  The original Deaf Access Program^TM primary care services of Dr. David Ebert moved to Access Community Health Network from Mount Sinai Hospital several years ago. The major portion of these primary care services are based in the following Chicago offices of Access Community Health Network, which makes possible a variety of improvements to services:
